Crowley has Lisa and Ben kidnapped as a warning to Dean not to meddle in his plans. The brothers summon Balthazar for help but to no avail. Their investigation takes them and Bobby to the world of H.P. Lovecraft, who supposedly wrote a manuscript describing Purgatory shortly before his death. They discover that Lovecraft invited guests for a first-hand look into another dimension during a dinner party, where all of them eventually ended up dead or insane.

Sam begs Castiel for help, who is enraged that Crowley threatened Dean with his loved ones. Balthazar meets with Castiel and tells him that he knows that he is in league with Crowley, but agrees to help Castiel to the end. Castiel saves Bobby speaks with the sole survivor of the Lovecraft party back in 1937 to know more details, but inadvertently finds out that the old man’s mother is Ellie Visyak (who Dean acquired the dragon-slayer sword from back in “Like a Virgin”), who is actually immortal and originated in Purgatory. She explains to Bobby that she fell through the cracks from the opening of dimensions and has been hiding out on Earth ever since. She refuses to tell Bobby how to open the gate to Purgatory or accept his help to protect her.

Meanwhile, the brothers get help from Balthazar who teleports them to where Lisa and Ben are held. While the brothers are split up, Sam is knocked out while Dean fights his way to Lisa and Ben. He discovers that Lisa is possessed by a demon who stabs her body to convince Dean to stop the exorcism spell. However, Dean is undeterred and exorcises the demon. He then carries Lisa all the way outside and reunites with Sam and the group heads for hospital. Castiel arrives to heal a dying Lisa. Castiel never wanted her nor her son involved. Dean then asks for another favor and Castiel also wipes clean all memories of Dean from them. Dean although grateful for his act remains unchanged about Castiel’s goal. Later, Ellie prepares to leave her safehouse but Castiel arrives and captures her.

Sam and Dean try to keep themselves busy by following up a new case where people are dying from bizarre and creative deaths (i.e., being decapitated by a garage door or having a photocopy machine choke a woman with her scarf). They determine there are no spirits or monsters involved, and remain at a loss. Inexplicably, Ellen (very much alive) is Bobby’s wife. She looks up the history of the victims and traces their family trees and discovers their ancestors were all immigrated to the United States in 1912 on the Titanic, a ship that none of them have ever heard of. Also inexplicably, Sam researches the Titanic and learns how it didn’t hit an ice berg and all the individuals passed safely by. He finds an old photo and notices the first officer of the ship is none other than Balthazar. The brothers call Balthazar over who reveals the true story of the Titanic – apparently, since Balthazar hated the movie he went back in time, saved the ship from hitting the ice berg which created a chain of reactions that led to Ellen being alive and no one knowing the uninteresting history of the Titanic. The cause of the bizarre deaths is revealed to be by one of the Fates sisters of Greek Mythology, Atropos who is killing the descendants of the Titanic survivors. Atropos attempts to kill Sam and Dean due to resentment of them escaping death from her and stopping the Apocalypse, though Castiel saves them. Castiel confronts Atropos and discovers how angry she is that no one takes her seriously anymore and that when she went to Heaven, everyone ignored her because of her failure to kill the 50,000 people on the Titanic. Right before Castiel and Balthazar decide to kill her, Atropos warns him that killing her will spark her sisters to kill them and Sam and Dean included. She tells Castiel (who convinced Balthazar to save the Titanic) that saving the 50,000 souls won’t help him when he needs them in his battle against Heaven. She convinces Castiel to tell Balthazar to go back and resink the Titanic, prompting the normal history to resume its pace, Atropos to regain her respect and Ellen is once again deceased.

Sam receives a mysterious text message giving him coordinates to a small town in Rhode Island. Upon further research, the brothers discover that three women had gone missing in the past week, prompting Sam to want to go investigate. Dean, however, is cautious, and believes that the text and the otherwise unknown location is a trap. Eventually, however, he concedes and the two head to the town. Upon arriving, they discover that Samuel and Sam had been in the town before, working on a hunt where men had gone missing. However, as time progresses in the town, Sam’s memories of his prior visit begin to surface, such as beating a suspicious police officer and having sex with various witnesses (who were now the victims). Sam is even arrested by the officer who he had beaten, and then confronted with the local sheriffs wife, Brenna Dobbs. Sam professes that he has no recollection of what happened, and Brenna frees him from jail. As the investigation continues, Sam realizes that he and Samuel had originally hunted an Arachne in the town, and Sam used Sheriff Dobbs as bait to lure in the creature. Upon finding Dobbs and the other abducted men, Sam successfully kills the Arachne and proceeds to kill the abducted men, as he believed that the men were poisoned and were beyond help. It is later shown that the Arachne had not captured the men to feed, but to propagate the species, turning Dobbs and the other victims from a year ago in Arachnes as well. Dobbs reveals himself by baiting Sam and Dean, and trapping them in webbing. While explaining what happened to him, and the fact that the women he kidnapped were all Arachnes and scattered everywhere, Dean uses a shard of glass to cut the webbing and attack Dobbs. Brenna, screaming for Dobbs to stop, frees Sam with a machete. Sam then proceeds to decapitate Dobbs. Sam then tries to apologize to Brenna, but she does not wait to hear him, and goes into her house. As Sam and Dean pack their belongings, Dean once again professes that Sam’s actions in the past year were not his own, and that Sam cannot keep unlocking memories for fear that the memory block Death put in Sam would fail. Sam still remains guilt-ridden, and wishes to continue finding out what he did so he can make amends. Suddenly, Sam collapses and convulses in seizures, and Sam’s soul is seen burning and screaming in pain.
